DAACCS’ personalised services provide everyday supports to assist you to live the life you choose. We understand that every person is different and we are here to support your individual goals and outcomes. We can support you to live as independently as possible, in whatever environment you choose. At home, in the community, at school or work, for business, health or leisure, we provide one-on-one support, seven days a week.

We firmly believe that you are the expert of your life and our role is to support you to experience a fulfilling life in the way you choose to live.

We believe that working in a partnership with families makes a big difference. Our commitment to you is that we will walk along with you in your journey to help you achieving your short and long term goals. Our Case Coordinators are professional and have many years of experience in the Disability sector in addition to relevant tertiary qualifications in social work/health or disability.

IF YOU HAVE FUNDING ALLOCATED THROUGH THE NATIONAL DISABILITY INSURANCE SCHEME (NDIS), DAACCS CAN WORK WITH YOU TO HELP YOU IMPLEMENT YOUR NDIS PLAN.

 

NDIS PLANNING/REVIEW

 

All individuals have different needs so no two plans are the same. Our dedicated staff will work with you to work out your needs, choices and short and long terms goals to get you ready for your Plan Review.

 

SUPPORT CONNECTION, COORDINATION AND TRANSITION PLANNING:

 

DAACCS staff play a very important role in connecting NDIS participants to local services and resources. We have a dedicated caring team who are friendly, innovative and active in the community – and ready to assist you with understanding how you can use your NDIS plan to meet your goals and live the life you choose.

DAACCS staff work together with you to;

  •  develop a detailed support plan that matches the goals identified in your NDIS plan and how together we can support you to achieve these goals
  •  manage resources effectively to get the best outcome from your plan
  •  work with you to plan and reach short, medium and long-term goals as you transition through different life stages.
  •  work collaboratively with you to personalise your supports and match you with the right staff and services.
  •  coordinate service providers and support
  •  liaise with government agencies including NDIS
  •  identify and source suitable accommodation and respite options
  •  link you into work and other community activities
  •  coordinate various therapy services
  •  coordinate health and medical specialists.

DAACCS provides personally designed Support Coordination and Case Management services to identify and manage supports and services for you to maximise opportunities and independence.

DAACCS will help you achieve your goals by sourcing the services and supports that meet your identified needs and goals. This includes providing you information in a transparent way and providing support to contact providers and develop service agreements.

RESPITE CARE

Currently there is no item listed under NDIS specifically as ‘respite’, which has caused confusion.  However, respite is available but in ways you may not be used to describing, such as:

  • Short term accommodation and assistance

  • Assistance in living arrangements (host family/alternative family) solution

  • Assistance with self-care overnight (different levels)

Care for those who care the most. Respite care allows carers to take a break from the demands of full time care. DAACCS can provide you with the support you need in the comfort of your own home or assist you to access alternative models of support.

DOMESTIC ASSISTANCE

We help those who want to live independently by providing support for a broad range of domestic tasks.

 

SKILL DEVELOPMENT

Assist you to develop your skills like personal hygiene, Independent living skills and travel skills etc. at your pace and the way you want.

 

SOCIAL SUPPORT

Support to individuals with disabilities that enables the individual, their families and carers to have choice and control over the support and services they choose.

 

MENTAL HEALTH SUPPORT

Individualised support for people with a mental illness or psychosocial disability, focusing on their strengths, positive mental health and sustainable connections to communities.

 

AGED CARE

We deliver a broad range of services to elderly people, all based on compassionate, collaborative care, delivering wellness with dignity.
