The Commonwealth Home Support Programme (CHSP) is a government-funded initiative designed to aid older Australians by offering entry-level support services, allowing them to live independently and safely at home. DAACCS partners with you to deliver CHSP services, ensuring you can live comfortably in your own home.

What is Commonwealth Home Support Programme (CHSP)?

The Commonwealth Home Support Programme (CHSP) is a government-funded initiative that helps older Australians live independently and safely at home by providing entry-level support services. Eligible individuals, typically aged 65 or older (50 or older for Aboriginal or Torres Strait Islander people), can receive services such as meal preparation, personal care, nursing, therapy, home modifications, maintenance, and social support. Costs are subsidized but may vary by provider and service type. An aged care assessment determines eligibility and specific support needs.

What's the process of applying for CHSP?

All applications for government-funded aged care services must be submitted through the My Aged Care government portal. Access Australian aged care information and services | My Aged Care

Check your eligibility and apply

Step 1. Apply for CHSP:

Step 2. Describe your level of need

If you are assessed as having:

Low-level needs – The Regional Assessment Services (RAS) will contact you to arrange an in-home, face-to-face assessment for the Commonwealth Home Support Programme (CHSP).

Higer, more complex needs – The Aged Care Assessment Team (ACAT) will reach out to schedule an in-home, face-to-face assessment for a Home Care Package (HCP).

Complete your face-to-face assessment

Step 3. Your RAS assessment

The assessor will inquire about your daily activities, needs, and preferences. You will discuss or demonstrate:

  • What you can do well
  • What you need help with
  • What you would like to be able to do

Step 4. Receive your assessment outcome If you are: Approved for CHSP…

Your assessor will:

  • Collaborate with you to create a personalized home support plan.
  • Provide you with a referral code.

Use this referral code when contacting DAACCS.

Organise and manage services

Step 5. Discussion with our Aged Care Experts, we will talk about: 

  • Your needs and goals
  • Available services
  • Cultural and language preferences
  • Our staff
  • Costs and fees

Step 6. Review your agreement

We will provide you with a formal agreement that details:

  • Costs and fees
  • How your services will be delivered

Once you agree with the terms, your services can start.

 

Step 7. Continue managing your services Payment

Each month, we will send you an invoice for:

  • Services you used
  • Payments due

 

Am I eligible for CHSP?

Eligibility for this program depends on your age and support needs.

If you find daily tasks challenging and believe that some assistance could enhance your health and wellbeing, the CHSP might be suitable for you.

To be eligible for this program, you must be:

  • 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people), or
  • 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people), with a low income and either homeless or at risk of becoming homeless.

An aged care assessment will determine your eligibility for the CHSP and the specific services you can receive.
