A Home Care Package is a government-funded program designed to help elderly people to access care services within the comfort of their own homes. It can be used to access a wide arrange of support services, from assistance with household chores, personal care to transportation for appointments.

What is a Home Care Package?
The Home Care Package program, established by the Australian Government, helps older Australians live independently at home. It offers flexible, personalised support based on individual health care needs. Funding is provided after a formal assessment and can include services to help a person stay:
Healthy and independent with personal care, nursing, and Allied Health support
Comfortable and safe at home with cleaning, gardening, home maintenance, and nutritional support
Engaged and connected to the community with transport and social support services
Eligibility is determined by the Aged Care Assessment Team (ACAT). If approved, you will receive funding based on your care needs to help you remain at home. The program offers four levels of funding.

Level 1 | Basic care needs – $10,271.10 a year |
Level 2 | Low care needs – $18,063.85 a year |
Level 3 | Intermediate care needs – $39,310.50 a year |
Level 4 | High care needs – $59,593.55 a year |

How to apply for a Home Care Package?
- Find out your eligibility – Apply for an assessment with My Aged Care, which can also be done over the phone.

- If you are eligible, you will be scheduled for a visit from the Aged Care Assessment Team (ACAT) for an assessment.
2. Have your ACAT assessment – An ACAT assessor will evaluate you either at home or over the phone to determine your level of care and priority.
3. Receive your assessment outcome
- My Aged Care will mail you a letter with the following information:
- Your eligibility for services
- The services you’re eligible to receive
- Additional supporting information
You will be placed in the National Priority System until a package is assigned. This system prioritises the allocation of Home Care Packages based on individual needs and circumstances, regardless of location.
4. Receiving your package and referral code
- Once assigned a package, My aged Care will send you a letter with a referral code.
- Contact your chosen Home Care Package provider.

5. Begin managing your HCP with DAACCS
- Start receiving your care and services!
Am I eligible for HCP?
- 65 years or older, or 50 years or older and identify as an Aboriginal and Torres Strait Islander person, or 50 years or older and on a low income, homeless or at risk of homeless
- Still living at home
- In need of help at home to continue to live independently
